Paragon is our third-party administrator (TPA) who processes and pays claims on behalf of Betenbough Companies’ healthcare plan. Paragon also assists in both consolidating and ensuring accuracy in billing with healthcare claims. As our TPA, they issue ID cards to policyholders, which must be shown to the provider prior to medical visits or procedures. Using the phone number listed on the card, your provider will call Paragon to verify coverage.
Your employer is self-funded for your health benefits. Your claims are “processed” by Paragon and “paid” by our Company once your deductible has been met.
Prior to your doctor visit, you should make sure your doctor has been submitted as a ClaimDOC provider (steps listed above). Once you arrive to your doctor’s office, they should call Paragon using the number on your ID card.
Nurse Sam is listed as a contact card if you have a Betenbough issued company cell phone. Otherwise, you can reach her directly through the Medxoom app, by calling her at 806.200.6684, or by emailing her at firstname.lastname@example.org.

ClaimDOC is an open-access coverage plan, meaning there are no out-of-network penalties or restrictions. All claims are processed and paid at an in-network basis, reducing the members’ financial responsibility, while not limiting your healthcare choices. With your help, ClaimDOC will establish a relationship with desired providers in advance of any needed care.
Prior to a healthcare visit, we need your help by asking you to submit a Provider Nomination Form by visiting this link, and using our company-specific PIN: CD50266. You will then select “Nominate Your Provider” and complete the steps to register yourself and/or desired dependents, as well as the provider and clinic information.
